Please note: All accompanying documents to the application must be postmarked by December 15, 2015 in order for the application file to be complete. Any application file not complete by December 15th will not be considered.
When the application is available, you will be asked for your contact information, emergency contact information, education and work history.
You will be asked to upload a personal statement/essay (750-1000 words) about yourself, your interests, why you chose to pursue Physician Assistant as a career and your future plans as a PA. The personal statement should communicate your motivation, goals, circumstances, and experiences related to a career as a Physician Assistant.
Official transcripts and official Test of English as a Foreign Language (TOEFL), if applicable, should be mailed to the Sullivan University PA Program, 2100 Gardiner Lane, Louisville, KY 40205. All applicants whose native language is not English, must submit TOEFL scores to demonstrate basic English proficiency. Applicants native to Australia, Canada, Ireland, New Zealand, and the United Kingdom will be presumed to be proficient.
The Recommendation Form and Direct Patient Contact Verification Form should be downloaded, completed by the appropriate individuals and also mailed to the Sullivan University PA Program, 2100 Gardiner Lane, Louisville, KY 40205.
Finally, you will be asked to submit an online payment in the amount of $100 for the application fee.
